Peacock Acres Non-Profit is seeking a Transitional Housing Program Plus Foster Care (THP+FC) full time case manager.

The THP+FC Case Manager is the overseer of all THP+FC residents; the primary responsibility of the manager is to provide intensive/therapeutic case management for Transitional Age Youth (TAY) between the ages of 16-24, who are in foster care or extended foster care. In addition, the case manager will play a supportive role to other staff in helping to meet the needs of all residents at Peacock Acres.

Key Responsibilities
• Build rapport and a strong foundation of trust with TAY residents between the ages of 16-24
• Work a full time (40 hours per week), afternoon/evening schedule of Tuesday through Friday, 12-8pm and Saturday from 9am-5pm or 12pm-8pm, depending on youth schedules and needs. A rotating schedule of one Friday per month from 9am-5pm will be required so the manager can attend meetings with social workers and other DSES representatives
• Check in with residential House Parents to monitor behaviors and progress at home; attend monthly House Parent meetings
• Perform intakes for new residents and create and update Transitional Independent Living Plans (TILPs) with each youth
• Monitor and initiate educational plans; provide educational support (time management, note taking, tutoring, etc.); monitor grades and educational outcomes; assist clients with college preparation or vocational training enrollment
• Assist with employment readiness, job searching, resume building, interview skills, etc.
• Implement rules and disciplinary actions when needed (using a therapeutic approach)
• Confer with social workers and CASAs (Court Appointed Special Advocates) to determine the progress and well being of residents
• Track all medical needs and appointments for residents
• Provide transportation to appointments, work, and school (when necessary)
• Maintain and create active client files with weekly case notes, SSN, California ID cards, medical cards, court reports, etc.
• Be aware of emerging emotional problems and provide counsel and appropriate intervention
• Communicate with TAY staff on behavioral changes or schedule changes of program residents
• Complete and submit proper paperwork requests to Peacock Acres Office Manager in a timely fashion in order to further meet the needs of TAY residents
• Attend weekly TAY staff meeting and weekly Peacock Acres staff meeting
• Assist with program development and activity/event planning

• A Bachelor's degree from an accredited institution in Behavioral Sciences or a similar field (psychology, social work, sociology, counseling), with at least 1-2 years experience working with at-risk youth (both male and female) in a nonprofit or community setting
• Must possess a valid California Driver's License and have a good driving record
• Able to satisfy the Department of Social Services and Department of Justice fingerprint clearance requirements through Community Care Licensing
• Have access to a clean DMV printout, a reliable vehicle, and the ability to drive a company vehicle
• Proof of negative TB test
• Must attend Red Cross/CPR certification within one month of hire (organization will sponsor training)
