Case Manager Veteran's Services (Salinas)

We are currently seeking a Case Manager for our Supportive Services to Veteran Families (SSVF) and other housing assistance programs. Veterans are strongly encouraged to apply.

Job Summary: The primary objective of case management is to ensure that participants receive the assistance needed to attain and maintain sustainable permanent housing. Case Management services include consulting with participants to conduct needs assessments; assessing financial and other needs; recommending appropriate levels of assistance; working with participants to create individualized housing stability plans that include participant specific goals; connecting participants to other services and benefits for which they might be eligible; and monitoring participant progress from the time of intake until program exit.
Provide intake and assessment for all applicants
Assess functional and social needs of clients in order to develop, implement and monitor service plans leading to an increased level of self--sufficiency
Determine and access appropriate community, state, federal, health and social resources
Provide Case Management office and in-home visits on an ongoing basis as needed by clients
Maintain client confidentiality
Provide outreach to community regarding resources available
Document and maintain in a timely manner, complete and confidential case notes on each individual or family client interaction.
Maintain complete and accurate HMIS entries of all services within the timeframe.
Ensure that all federal, state, agency and /or funder guidelines are met.
Other duties as assigned by the Program Manager or Executive Director.

Familiarity with local service providers, and community resources is strongly preferred.
Education, Experience, and Certification Requirements:
Bachelor's degree in Human Services or related field
HMIS , and/or SSVF, HPRP or other housing program or related experience preferred
Minimum of three years' experience in Case Management or related Human Services
Valid state Driver License at time of hire (Good driving record).
Use of own vehicle with current insurance.
Must have no criminal record: background and credit checks will be conducted.
Hours of Work: 40 hours per week M-F; with occasional weekend and evening hours as scheduled.
Salary: BOE + health and vacation benefits.
